The Face Dementia Practice Improvement Program supports general practitioners, practice nurses and their teams to improve identification, assessment and management of dementia.

The General Practice Training Program was developed in collaboration with Dementia Training Australia, Western Victoria Primary Health Network and Western Sydney Primary Health Network (WentWest PHN), general practice teams, and people with dementia and carers.
